The application form is an official document required for beginning the procedure of recognition of a foreign higher education qualification. The form is to be legibly and completely filled out. The applicant acknowledges liability for data accuracy with his/her signature on the application form. The recognition procedure cannot officially commence unless and until the form is accurately and completely filled out.
There are three official application forms:
1. Application for professional recognition of foreign higher education qualification with employment in the Republic of Croatia as its purpose [professional recognition]

If you change your address, first or second name during the period of recognition procedure, please inform the National ENIC/NARIC Office in written form.
